Comprehensive Appointment

The cost for the initial evaluation of the long-form history and previous records is $45, which can be paid at the time of scheduling your full comprehensive appointment.  This cost is for the preliminary review of your long-form history and your current/previous medical records only, not the comprehensive appointment. 

After you have made your request, we will contact you to discuss the initial evaluation of your form and records, and we will confirm the date and time of the requested comprehensive appointment.
