Overview
Your first visit to our wellness center will establish a vital foundation for your health. We offer two types of appointments for our new patients, and the details are outlined below. The charge for new patient intake and initial evaluation of records for comprehensive appointments is $45 and can be paid once the forms have been filled out, medical records have been sent and a request to schedule a full evaluation has been made. REQUEST
Immediate Appointment
Immediate appointments are only available for acute conditions such as eye infection, ear infection, strep throat, etc. If you do not have an acute condition, please use the comprehensive appointment request option. For the immediate care appointment, you will be filling out the short-form medical history form only.

Comprehensive Appointment
Comprehensive appointments are available for all patients. Please note that once we receive your long-form medical history form and the medical records from your current/previous medical providers, we will be able to schedule a full comprehensive appointment. The full comprehensive appointment will take between 1 hour and 1-1/2 hours.
